RMA Number Request Form

Dear Customer,

Use this form to request for an Authorization to return merchandise.  Upon receipt and approval, StockerYale will provide you with your RMA number via Email.  You must include your RMA number when returning merchandise to StockerYale.

Instructions:

  1. Please fill in your company and contact details.
  2. Fill in your product details in the "Add Item" boxes then click on the "Add this item" button. Repeat this step to add more items.
  3. Click on the "Submit RMA Form" button when you're done.
